Medication errors in critical patients during medication reconciliation: analyses and clinical management

Medication errors (ME) are frequent in the admission of patients to ICU and can be identified prevented through medication reconciliation (MR). Our aim was evaluate incidence, type severity MEs associated factors, during MR ICU. This is a prospective, analytical approach, performed private hospital, where MRs were evaluated from April June 2016. The SPSS Stata programs used analyse data. Logistic regression determine factors with MEs. 136 patients, whom 126 (92.6%) drugs regularly. incidence 16.3% (95% CI 11.5-21.2). main classes involved those acting on nervous cardiovascular systems. There 128 pharmaceutical interventions (acceptance: 71.1%). Regarding severity, 65.5% (n=80) reached patient, but there no harm. risk for were: age ?60 years, number comorbidities >1 previous use ?9. found significant association age, polymedication alert need specific attention prevent most susceptible patient groups.
